New Patients:

 

Thank you for choosing Renaissance Health and Surgical Associates for your medical care. We look forward to meeting you. 

 

As a new patient, we allow more time for your visit with your provider. Because our time is as valuable as yours, we ask that you call us 24 hours in advance if you have to reschedule or cancel your appointment.

 

Before your first visit:

  • Verify the provider your are seeing is contracted with your insurance plan.

  • Ask your insurance if you need a referral from your primary care physician.

  • Make payment arrangements if you do not have insurance coverage.

  • Make a list of questions or concerns you wish to discuss with us.

On the day of your appointment, please bring:

  • Your insurance card

  • Completed New Patient forms (see forms section to print)

  • A list of all your current medications

  • A list of questions or concerns you may have

  • Any copay or deductible amount applicable

Office Visits:

 

Please arrive to the office 15 minutes before  your appointment. We understand traffic can slow you down and we do allow a 15 minute "grace" period. If you are late, you may be asked to reschedule your appointment. 

 

Since many of our services are driven by your insurance plan, it is imperative to bring your insurance card with you to your appointment. If you do not have your insurance card, you will be asked to reschedule your appointment or you may have the option of paying for the services at the time of the visit.

 

Copays and deductibles are collected at the time of your visit as well as any outstanding balance on your account. Please be prepared to pay these amounts when you arrive at the office. Click here to see accepted payment types.

 

If you are visiting for an annual exam, this does not include treatment for existing/new medical problems or prescription refills. If you request or receive prescription refills or receive treatment for any other medical problem during your annual exam, this will be charged in addition to your annual exam. It is your responsibility to know your insurance benefits before a service is performed and if our physicians are participating with your insurance plan. Please notify us if you are visiting for an annual exam or a "Welcome to Medicare" exam. Please see this important information about an annual exam.

 

You may shorten your time in our office by printing out the patient forms listed in the forms section. Please complete them and bring them with you to your visit.

Lab Work:

 

For your convenience, we have a certified phlebotomist from Associated Pathologists located in our office for performing routine blood work ordered by our physicians. Some routine blood work may be done during your office visit, however you may be asked to come back another day if your labs require fasting or other special instructions. You do not need to have an appointment for blood work. You may come any time during our regular hours.

 

Associated Pathologists is a pathology group providing anatomical, clinical, and administrative services to hospitals, multi-specialty clinics, surgery centers, physician offices and other clinical reference laboratories since 1965.

Phone Calls:

 

We receive a large volume of phone calls, especially on Mondays. We make every effort to have a live person answer your call, however you may reach our voice mail. If this is the case, please leave a detailed message of:

  • your name and date of birth

  • your concern

  • your provider

  • pharmacy phone number

  • contact phone number to return your call

All voice mail boxes are checked regularly and messages are handled during business hours.

Medication Refills:

 

Please call our office during business hours and leave a voice mail for medication refills. Your request will be processed within 24 hours or less. Please provide the following information:

  • Patient's name and date of birth

  • Drug name, dosage and frequency taken

  • Pharmacy name and phone number

Calls received after 4:00 pm M-TH or after 11 am on Friday will be processed the next business day. Please do not call after hours for routine medication refills.

Confidentiality:

 

We consider your medical information strictly confidential. Your medical records can only be released with your written permission. If you would like your records sent to another physician, we will require a written authorization from you.

Patient Forms:

 

At each visit to our office, you will be asked to complete registration forms which will provide us with basic information about you. In addition, you will be asked to complete a medical history questionnaire which will provide us with valuable information about your medical background. It is important that you fill these out completely and accurately so we can provide you with the best care possible.

 

To save the amount of time spent in our office, we recommend that you download the patient forms from this page, complete them, and bring them with you to your visit.
